Position Overview

Under the general direction of the Manager or designate, the Utilization Facilitator is responsible for:

  1. Facilitating and coordinating the movement of patients through the admission, transfer, and discharge process as required.
  2. Liaising between internal and external agencies on matters related to patient movement.
  3. Participating in the development of guidelines and educational strategies related to patient admission, transfer, and discharge.
  4. Participating in decision-making that results in an effective admission process and the optimal utilization of facility resources.

Experience

  • Minimum 4 years recent nursing experience.
  • Recent acute clinical experience preferred
  • Demonstrated ability to function in a pivotal role, as facilitator in the patient admission/discharge/transfer process between patient, family, physician, Clinical Manager, members of the multidisciplinary team, other programs, community resources, etc.

Education (Degree/Diploma/Certificate)

  • Baccalaureate degree in Nursing required.
  • Master’s degree preferred.
  • Educational preparation in utilization an asset.

Certification/Licensure/Registration

  • Current registration with CRNM required.

Qualifications and Skills

  • Demonstrated ability to initiate and maintain effective resource contacts.
  • Demonstrated ability to work with all levels of staff (direct care, multidisciplinary team, management).
  • Demonstrated ability to analyze and problem-solve.
  • Excellent interpersonal and communication skills.
  • Demonstrated ability in utilizing computerized applications, such as Microsoft Word and Excel.

This maternity/parental term may expire sooner than the date indicated, subject to written notice of a minimum two (2) weeks, or one (1) pay period, whichever is longer.

 

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
