Position Overview

Under the general direction of the Program Director, Medicine or the Associate Executive Director, Clinical Programs, and in close collaboration with the Medicine Program Physician Bed Manager and/or relevant Program Team Managers/Program Directors, and while demonstrating a commitment to the Mission, Values and Management Philosophy of St. Boniface General Hospital, the UTILIZATION FACILITATOR is responsible for:  facilitating and coordinating the movement of patients through the admission, transfer and discharge process primarily within the Adult Medicine, Family Medicine and/or Emergency Programs, as well as other clinical programs as required; liaising between internal and external programs and agencies on matters related to patient movement; participating in the development of guidelines and educational strategies related to patient admissions, transfer and discharge; participating in decision making that results in an effective admission process and the optimal utilization of Program and facility resources; and performing other related duties as assigned.  

Experience

  • Minimum four (4) years recent related nursing experience in Acute Medicine and/or Geriatric Medicine required

Education (Degree/Diploma/Certificate)

  • Bachelor’s Degree in nursing required and/or an equivalent combination of education and experience will be considered
  • Education preparation in-patient admissions, transfer and discharge an asset

Certification/Licensure/Registration

  • Current CRNM registration required

Qualifications and Skills

  • Knowledge of computer systems and proficiency in the use of spreadsheet, word processing and e-mail software applications required
