Job Summary:

 

Reporting to the Program Manager – Patient and Family Support Services, and with guidance from the Clinical Lead, Psychosocial Oncology Program; the Social Worker will primarily support pediatric patients living with hemoglobinopathies (e.g. Sickle cell anemia) by providing assessment, navigation, and case management services to patients and individuals who are eligible.

This role will provide a variety or resources and support to patients and families which involves discussing challenges that may impact access to care and treatments. This position will plan, seek, advocate and monitor requests by liaising and collaborating with internal and external departments, social services, health organizations and community resources, on behalf of the patient.  The Social Worker will develop relationships and work in collaboration with patients, families, the Hematology and Pediatric Oncology programs, nurses, physicians, pharmacy and other internal programs and external organizations to provide support for patients

 

Qualifications:

Required:

Education

  • Post secondary degree in Social Work from an accredited educational institution

Experience

  • 2 years of related social work experience working with children and families in the last 5 years

 

An acceptable equivalent combination of education and experience may be considered

 

Skills and Abilities

  • Expertise and experience with interprofessional collaboration, psychosocial assessment, consultation, and team building
  • Ability to provide standard Social Work services including assessments and consultation, case management, counseling, crisis intervention and referral to appropriate resources
  • Extensive knowledge of existing community resources and agencies providing relevant services to children and families in both rural and urban communities
  • Knowledge of Child and Family Services
  • Demonstrated ability to promote participation, collaboration and partnerships
  • Demonstrated professional, community and public education skills
  • Demonstrated skills in areas of crisis intervention, abuse investigation and assessing high risk situations. Understanding of child welfare act, standards and regulations
  • Ability to adapt readily to changing demands and situations
  • Demonstrated understanding of health equity, equity-denied populations and social determinants of health
  • Proficiency with Microsoft Office programs as well as other software (E.g. ARIA, EPR, RL6, etc.)
  • Excellent assessment, interviewing and counseling skills
  • Good organizational, time management and prioritizing skills
  • Advanced communication skills, both written and oral
  • Demonstrated ability to interact with people in a sensitive, tactful, diplomatic and professional manner
  • Ability to work in an ethical manner and ensure integrity and a high level of confidentiality
  • Ability to conduct practice in accordance with related policies, and legislation, including legislated confidentiality requirements of a health care setting

Asset:

  • Experience working with immigrant populations
  • Ability to speak French is considered an asset

 

Required Licenses, Registrations, Designations

  • Must be registered or eligible for and maintains licensure to practice as a Social Worker in the province of Manitoba.

 

Physical Demands and Working Conditions

  • May be required to hold a valid Manitoba driver’s license and have access to a reliable vehicle.
  • May encounter aggressive and/or agitated client/visitors/staff
  • May be occasionally required to work extended hours

Other

  • Must have satisfactory employment record
  • Must be legally able to work in Canada
