Position Overview

River Park Gardens is an 80‐bed facility located in South St. Vital. River Park Gardens is owned and operated by the Winnipeg Regional Health Authority. The philosophy of care is based on a resident and family‐centered approach that is grounded in mutually beneficial partnerships among healthcare providers, residents and families.

This position will function in accordance with River Park Garden’s mission, vision, values; objectives, policies and procedures; Manitoba Health standards; Accreditation Canada standards; and provincial and regional policies/guidelines.

Under the direction of the Director of Care (DOC), River Park Gardens, the incumbent is responsible for:

  • Providing professional, skilled psychosocial assessment and interventions to individuals, families and groups; providing a range of psychosocial interventions that are aimed at assisting clients, families and groups cope with and adapt to health-related issues, using the healthcare system and other social systems effectively and efficiently; demonstrating leadership in assisting the healthcare team complete transitions.
  • Providing leadership in the admission process of new residents as well as the coordination and leadership of post-admission, quarterly and annual care conferences.
  • Participating as a member of the healthcare team in interpreting social, psychological, emotional and family problems to the team; maintaining excellent working relationships with team members and participating in team planning and problem-solving; performing a variety of administrative functions as required.
  • May be requested to assist in providing instruction to students and new staff. This position is responsible for the management of resident/family/caregiver concerns and advises and/or seeks assistance from the DOC as required.

Experience

  • Two (2) years’ experience in a residential or long-term care setting preferred.
  • Experience in counseling and group work with the elderly and families an asset.

Education (Degree/Diploma/Certificate)

  • Bachelor’s degree in Social Work (BSW) from a Canadian Association for Social Work Education (CASWE) accredited school/faculty, or a Provincially authorized post-secondary program, or the approved equivalent if internationally educated.

Certification/Licensure/Registration

  • Must be registered with Manitoba College of Social Workers.
  • May be required to hold a valid Manitoba driver’s license and have access to a reliable vehicle.
  • Cardiopulmonary Resuscitation (CPR) Training requirements for this position shall be in accordance with the Employer policy.
  • Retains membership and participates in the Manitoba Association of Personal Care Home Social Workers.

Qualifications and Skills

  • Ability to establish and maintain good interpersonal relationships.
  • Must have effective written and verbal communication skills.
  • Ability to work cooperatively with other disciplines and demonstrate professional judgment and initiative.
  • Willingness to adapt to new concepts, techniques and best practices in the academic and clinical fields of healthcare.
  • Understanding of health and social welfare systems.
  • Ability to practice independently and with minimum supervision.
  • Demonstrated strong commitment to the profession of social work and the Canadian Association of Social Work (CASW) Code of Ethics and the Manitoba College of Social Workers Standards of Practice.
  • Must be organized in the workplace and determine clear priorities.
  • Computer literacy.
  • Assume the role of the privacy officer for the facility.
  • Chairs, co-chairs and attends facility committee meetings as required.
  • Chairs the Resident Council.
  • Coordinates care conferences and maintains a master care conference list. Participates in the development implementation and evaluation of the resident care plans.
  • Provides communication to the Office of the Public Guardian and Trustee of Manitoba regarding needs of the residents as applicable.
  • Assists residents and their representatives in the admission process.
  • Assists residents and their representatives in the discharge planning process as needed and assists in coordinating applications to alternate personal care facilities.
  • Provides end of life support to families and their representatives.
  • Documents resident information according to professional/legal guidelines and site policy and procedures.
  • Communicates and responds to resident and their representative's inquiries, concerns or complaints in a timely and responsive manner.
  • Works in collaboration with nursing management. Participates in areas of CQI and staff development as required.
  • Practices effective mediation and conflict resolution.
  • Attend monthly staff meetings, in-services and conferences as required.
  • Represents site on external committees as required.
  • Supports and adheres to the policies and procedures of RPG.

Physical Requirements

  • May encounter aggressive and/or agitated client/visitors/staff.
  • May be exposed to infectious diseases, blood and body fluids, toxic materials, noise, allergens, physical and emotional stress.
  • May be occasionally required to work extended hours.
  • May be required to work a variety of shifts including but not limited to days, evenings, and weekends.
  • May be required to participate in the after-hours on call rotation structure at River Park Gardens, which may include, but is not limited to the following duties:
    • Directs or assigns work; determines who does what.
    • Orders or authorizes overtime.
    • Calls employees to work.
    • Problem-solves emergent operational challenges.
    • Involves WRHA on call structure as required.

 

 

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
