The successful applicant may be eligible for an annual full-time incentive of up to $2,000 each year until 2025 as per MOU #31 of the current MNU collective agreement.
Position Overview
The Access Coordinator is a pivotal member of the cardiac care team and utilizes advanced cardiac skills and knowledge to: coordinate information; facilitate the movement of patients through the admission, transfer and discharge process; liaise between internal and external programs and external programs and agencies on matters related to patient movement; and assist referring physicians to access cardiac care. The incumbent facilitates physician to physician communication and manages the waitlist for cardiac care. This includes working in conjunction with the patient, cardiac specialist(s), referring physician(s) and hospital staff to monitor patients either at home or in hospital and triaging inpatients and outpatients by acuity level to ensure appropriate and timely access to cardiac care. The Access Coordinator is responsible for maintaining data bases and reporting statistical data. The incumbent provides patients and their families with information and support throughout their journey in receiving cardiac care. The Access Coordinator utilizes advanced skills and knowledge and performs at a high level of independence as a clinical practitioner, leader, consultant and educator. Practices are in accordance with employer policy and professional standards.
Experience
- Minimum of two years’ experience in the Cardiac Sciences field including interpretation of cardiac investigative reports, (e.g. angiograms, MIBI, GXT, echocardiograms) and patient education required.
- Satisfactory employment record required.
Education (Degree/Diploma/Certificate)
- Graduate from an approved School of Nursing required.
- Successful completion of the WCCNEP and/or an Advanced Assessment course or equivalent required.
Certification/Licensure/Registration
- Current registration or eligibility for registration with College of Registered Nurses of Manitoba (CRNM) required.
- Cardiopulmonary Resuscitation (CPR) training requirements for this position shall be in accordance with the Employer policy.
Qualifications and Skills
- Ability to function in a demanding and stressful environment.
- Advanced clinical knowledge in cardiac care.
- Expertise and competence in nursing practice, interviewing, counseling, teaching and group dynamics.
- Administrative and computer skills essential, experience preferred in word processing, spreadsheet, and database applications.
- Excellent communication and interpersonal skills.
- Commitment to care through a family centered nursing approach.
- Advanced clinical knowledge in area of expertise through reviewing literature and attending continuing education in-services, workshops and conferences.
- Ability to work independently but collaboratively with patients, families and multidisciplinary team members.
- Ability to establish and maintain effective working relationships with referring physicians and nurses and support team.
- Displays initiative, accountability, good time management, decision-making ability, leadership, strong organizational skills, and ability to manage competing priorities.
- Knowledge and skills in research and quality assurance.
- Demonstrated excellence in interpersonal and supervisory skills, including verbal, written, and organization skills.
