Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.

 

Position Overview

Under the supervision of the Executive Director in compliance with CRNM and CANO Standards of Nursing Practice, the Registered Nurse is responsible for reviewing, assessing and triaging patient referrals to ensure timely appropriate and coordinated care. This role serves as a key clinical link between referrers, patients, and care teams, using clinical judgment to determine eligibility, urgency, and next steps in the care pathway. This position is responsible for coordinating, monitoring, and supporting quality improvement and compliance activities at the site level. This role ensures that services, processes, and outcomes meet established standards, regulatory requirements, and organizational goals. The Quality Coordinator works collaboratively with site leadership and staff to promote a culture of continuous quality improvement and safety.

Experience

  • Minimum of 2 years related experience at a referral assessment desk or with a triage role.
  • Minimum of 3 years related experience in oncology, surgery, clinic setting ambulatory care, case management.
  • Experience in patient education.

Education (Degree/Diploma/Certificate)

  • Baccalaureate Degree in Nursing (or equivalent combination of knowledge, training and experience).

Certification/Licensure/Registration

  • Eligible for registration with CRNM.

Qualifications and Skills

  • Well-developed organizational and communication skills.
  • Experience working with an interdisciplinary team.
  • Computer knowledge and experience, Aria preferred.
  • Excellent problem-solving skills.
  • Ability to maintain confidentiality.

Physical Requirements

  • Ability to perform the essential physical requirements of the position.
  • Moderate physical effort required.
