Position Overview

The Palliative Care Coordinator functions as a key member of an interdisciplinary team within the Palliative Care Program. The Palliative Care Coordinator in consultation with other team members will have primary responsibility for determining eligibility of clients for the WRHA Palliative Care Program and its components based on established criteria. The coordinator will be responsible for ongoing case management of palliative care clients, including Home Care services. The coordinator will be the primary contact for palliative care team members regarding changes in client conditions and service needs.

Experience

  • Minimum three years directly related community experience in Palliative Care required.
  • Community experience an asset.

Education (Degree/Diploma/Certificate)

  • Graduate of an approved Registered Nursing education program required.
  • BN,BScMH is preferred.

Certification/Licensure/Registration

  • Responsible for maintaining and providing proof of registration with the College of Registered Nurses of Manitoba (CRNM).
  • Cardiopulmonary Resuscitation (CPR) Training requirements for this position shall be in accordance with Employer policy.
  • Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) preferred.
  • CNA certification in Hospice Palliative Care Nursing an asset.
  • Valid Driver’s License and use of a vehicle.

Qualifications and Skills

  • Familiar with philosophy and vision of the WRHA Palliative Care Program.
  • Knowledge of computer systems, word processing and email applications.
  • Demonstrated effective verbal and written communication skill.
  • Ability to carry out responsibilities of the position with minimal supervision and direction.
  • Proven ability to work as a member of an interdisciplinary team.
  • Ability to adapt readily to changing demands and situations including conflict resolution and negotiating skills.

Physical Requirements

  • May be required to work for long periods of time at a computer.
  • May be required to stand, bend and do lifting for extended periods of time.
  • May be required to answer phones for extended periods of time.
  • Available to work days, evenings, and weekend rotating shifts.
