Position Overview

Reporting to the Community Team Manager of the My Health Team, the Occupational Therapist will contribute to the assessment and care of clients of My Health Team partner clinics. The successful candidate will work in fee for service practices throughout the designated community areas and other locations as needed, with a base site at XXXXX.

My Health Team (Primary Care Network) is a collaborative partnership between independent primary care practices, community organizations and regional health authorities. Partners work closely with one another to plan, develop and provide enhanced local primary care services to their patient populations. This will also support the patient journey through the health system and across community and acute care.

As part of the My Health Team, the Occupational Therapist will work to their full scope of practice, in a collaborative/interprofessional team environment to improve the health and well-being of patients dealing with the consequences related to injuries, chronic diseases, developmental disabilities, mental health challenges, health inequalities, etc. The Occupational Therapist will work in both the clinic environment and the community (including but not limited to home) providing assessment and interventions.

The Occupational Therapist role will include consultation, collaboration with Team physicians and other partners, assessment, counseling, education and interventions directed toward client-centered goals considering occupational performance areas, and relevant performance components to help clients remain independent and safe in their home community environments. Relationship-building and collaboration with partner physicians/providers will be a significant part of this role as well as participating in service evaluation and further planning and development of the role.

Experience

  • Minimum of two years of directly related experience in a health care setting with preference for experience in primary health care services.
  • Experience working with persons with various health conditions (e.g., chronic diseases, developmental disabilities, mental health challenges, etc.) and in developing community linkages and relationships.

Education (Degree/Diploma/Certificate)

  • Entry-to-practice academic credential in Occupational Therapy from an accredited Canadian university program or the approved equivalent if internationally educated as determined by the College of Occupational Therapists of Manitoba (COTM).
  • Successful completion of relevant continuing professional education in designated clinical area preferred.

Certification/Licensure/Registration

  • Registration is required with the College of Occupational Therapists of Manitoba (COTM) with the incumbent’s name entered in the practicing register or in the provisional register as an examination candidate.  Where the incumbent is provisionally registered as an examination candidate, to maintain employment, he/she must pass the examination required by COTM during the next available exam offering to maintain employment with the Centre.
  • Cardiopulmonary Resuscitation (CPR) Training requirements for this position shall be in accordance with the Employer policy.
  • Membership in the Manitoba Society of Occupational Therapists (MSOT) and the Canadian Association of Occupational Therapists (CAOT) preferred.

  • Computer literacy as appropriate to the setting.
  • Ability to plan, implement and deliver care for individuals and groups.
  • Ability to engage and support patient through self-managed care, health behavior change, mental fitness, self-efficacy and resilience.
  • Understanding of fee for service family medicine work environment an asset.

Physical Requirements

  • Must demonstrate adequate strength, agility, motor coordination and dexterity for implementation of therapeutic techniques and manipulation of therapeutic activities as necessary.
  • May be exposed to infectious diseases, blood and body fluids, toxic materials, noise, allergens, physical and emotional stress.
  • May encounter aggressive and/or agitated clients/visitors/staff.
  • May be required to occasionally work extended hours.
  • May be required to work a variety of shifts including weekends.

 

 

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
