The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.
Position Overview
As a member of an interdisciplinary team, the Client Care Coordinator (CCC), Virtual Peritoneal Dialysis (PD) Unit provides comprehensive health care with an emphasis on PD care, PD education, chronic kidney disease management and clinical intervention. Develops and implements a care plan with the Licensed Practical Nurses (LPNs), the patients and evaluates success in meeting this plan. Provides ongoing service coordination and links patients with resources.
The CCC will determine patient eligibility for admission to the Virtual PD Unit and assess the need for PD care at home or in a personal care home. The CCC will supervise the LPNs employed in the Virtual PD Unit and coordinate the delivery of a broad range of PD services. Acts as a liaison and maintains communication with PD units and in-patient units when the Virtual PD Unit patients are admitted to or are provided services at Seven Oaks Hospital and St Boniface Hospital as well as other facilities involved with the PD patient group.
Under the general direction of the Program Director, the incumbent is responsible for:
- Actualizing the mission, vision, and values of the Winnipeg Regional Health Authority (WRHA) and the MB Renal Program (MRP).
- Providing expertise in the management of chronic kidney disease and the related conditions.
- Practicing aims to minimize complications arising from chronic kidney disease and PD, maintain optimal physical and emotional functioning and promote self-determination/management by the individual patient.
- Maintaining patient-centered care with a strong emphasis on an interdisciplinary team approach. The CCC is an integral member of the team, providing support to the maximization of each disciplines’ scope of practice.
- Working as a coordinator / navigator on behalf of the patient to ensure the appropriate links / referrals to other services that would enable the patient to achieve their health goals.
Experience
- Four years of recent, relevant experience and demonstrated competency in nephrology nursing is required. Relevant experience may include:
- Hemodialysis nursing.
- Peritoneal dialysis nursing.
- In-patient nursing with chronic kidney disease patients.
Education (Degree/Diploma/Certificate)
- Baccalaureate Nursing Degree is required.
- In addition, successful completion of the MB Nephrology Nursing Course.
Certification/Licensure/Registration
- Current College of Registered Nurses of Manitoba Registration (CRNM) required.
- Possession of a valid Manitoba Class 5 Driver’s License required.

Physical Requirements
- Lifting and carrying heavy equipment and supplies is required.
- Providing service in a wide variety of community settings under varying weather and environmental conditions is required.
- Access to a reliable motor vehicle suitable for all environmental conditions.
- Acceptable Child Abuse Registry check and Criminal Record check.
