Pensionable Salary Enhancement - Nurses in eligible classifications may be eligible to earn an additional $5.95/hour when working full-time hours within a prescribed six-month reporting period. (Effective April 1, 2025 – March 31st 2027.)
Position Overview
Under the general direction of the Manager Health Services and the clinical direction of the Medical Director, Manitoba Adult Heart Transplant Program, and while demonstrating a commitment to the Mission, Values and Management Philosophy of St. Boniface Hospital, the CARDIAC TRANSPLANT AND VAD COORDINATOR is responsible for: coordinating patient care, in collaboration with other members of the Multidisciplinary Team, for patients with advanced heart failure most likely seeking heart transplant and VAD procedures. This includes coordinating the functions and activities related to the Heart Transplant and VAD Program (for heart recipients and living donors); providing expert nursing care to patient recipients and their families pre and post-transplant; functioning as an expert and acting as a clinical and information resource to others both within and external to the facility on heart failure/transplant; participating in orientation and educational activities; participating in rounds; participating in the Heart Transplant Clinic; participating in clinical research; under the direction of the Medical Director, processing new referrals and initiating diagnostic and therapeutic investigations/interventions; and performing other related duties as assigned.
Experience
- Minimum of 4 years of recent clinical experience in cardiology, cardiac surgery, or critical care is required.
- Previous experience working with heart failure, transplant, or LVAD patients is strongly preferred.
- Experience in program coordination, quality improvement initiatives, or leadership roles within a multidisciplinary healthcare environment is preferred.
Education (Degree/Diploma/Certificate)
- Bachelor’s Degree in Nursing required.
- Post graduate course in I.C.U. Nursing, Advanced Assessment or recognized equivalents required.
Certification/Licensure/Registration
- Current CRNM Registration required.
- Active participation in professional associations e.g. Association of Registered Nurses of Manitoba (ARNM) preferred.
Qualifications and Skills
- Knowledge of computer systems and proficiency in the use of common software including word processing, spreadsheet, EMR, internet and email applications required.
- Ability to work independently and effectively in a multidisciplinary team setting.
- Strong verbal and written communication skills.
- Excellent organizational, interpersonal, and critical thinking abilities, with demonstrated problem-solving, leadership, and decision-making skills in high-pressure environments.
- Proven clinical competence.
- Strong teaching and mentoring capabilities.
- A commitment to self-development and the ability to pursue continuous learning opportunities.
