The successful applicant may be eligible for an annual full-time incentive of up to $2,000 each year until 2025 as per MOU #31 of the current MNU collective agreement.
Position Overview
The Mental Health Transition and Liaison Coordinator is a member of the acute Mental Health team within Victoria General Hospital who assist in care/treatment planning, consultation, system navigation, discharge planning and providing bridging services until appropriate resources are accessible.
The Mental Health Transition and Liaison Coordinator has a fundamental role in providing consultation and support to patients with mental health conditions who are receiving services in non-mental health patient care areas (e.g. medicine or surgery programs) at Victoria General Hospital.
Experience
- Minimum of 3 years of recent acute mental health experience required.
- Advance knowledge of and extensive experience liaising with mental health community resources.
- Effective networking and agency relations skills.
Education (Degree/Diploma/Certificate)
- Graduate of an approved School of Nursing or Psychiatric Nursing required.
- BScMH, BScPN or BN preferred.
Certification/Licensure/Registration
- Licensure/registration as per professional designation required.
Qualifications and Skills
- Comprehensive knowledge and understanding of the bio/psycho/social model of mental illness and recovery.
- Excellent organizational, communication and interpersonal skills.
- Comprehensive knowledge and advanced level expertise in care planning, clinical assessment, crisis intervention, brief treatment, service coordination and consultation.
- Able to use problem resolution skills to handle sensitive and complex issues related to multiple service involvement.
Physical Requirements
- May require evening and weekend work.
This term position may end earlier as outlined in your collective agreement.
This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
