Position Overview
Within the Winnipeg Health Region there are over 5,700 people who live and receive care and health services in long-term care facilities. The Long-Term Care program is an interdisciplinary team of individuals who work with Long-Term Care facilities to provide a continuum of clinical services designed to support the needs of people living with chronic health problems that affect their ability to perform daily activity. Although caring for the elderly is the main area of focus the program provides care and services for individuals across the entire age spectrum.
The program provides content expertise, evidence informed recommendations/guidelines, and initiatives designed to sustain and improve the quality and consistency of care provided throughout Winnipeg’s 39 long-term care facilities.
The successful candidate will be responsible to develop a relationship of support and guidance between WRHA Continuing Care and individual Personal Care Homes. Some key areas of focus are:
- Information gathering, analysis and support for action plan creation for operational issues as identified by Continuing Care leadership, RL6 reporting, Client Relations reports, Protection for Persons in Care Office, Manitoba Health Standards, etc.
- Completion of Manitoba Health PCH Standards visits as a WRHA Continuing Care representative.
- Onsite support for regional clinical, quality, and educational priorities.
- Resident-specific follow up related to requests for supplemental staffing.
- Review of specialized PCH bed capacity and utilization
- Communication of risks in a timely way to Continuing Care leadership.
- Flexibility for occasional evening, night and weekend visits.
Areas of knowledge essential to success:
- Personal Care Home operations ;
- Manitoba Health PCH Standards;
- Relevant legislation (e.g., PHIA, Protection for Persons in Care Act, Whistleblower legislation);
- RL6 Incident Reporting system;
- Adult education principles.
Experience
- Experience in program planning, development, evaluation and managing patient care services required.
- Expertise in long-term care preferred.
Education (Degree/Diploma/Certificate)
- Bachelor degree in Nursing required.
Certification/Licensure/Registration
- Responsible for maintaining and providing proof of active registration with the appropriate registering body.
- Valid driver’s license and access to a vehicle.
Qualifications and Skills
- Demonstrated proficiency with the use of Microsoft Office products (Word, Excel, PowerPoint) required.
- Knowledge of regional policies, procedures, protocols and guidelines.
- Knowledge of the Personal Health Information Act, Workplace Hazardous Material Information Systems (WHMIS), Principles of Routine Practices, and other legislated acts.
- Demonstrates effective communication and presentation skills and ability to communicate effectively both in oral and written form.
- Ability to interact and lead effectively as a member of interdisciplinary team.
- Problem-solves, organizes results and monitors overall operations of the Transitional Care Environment.
- Prioritizes workload, responds to simultaneous demands and plans, manages, and implements change.
- Demonstrates interpersonal, critical thinking, conflict resolution and decision-making skills.
- Ability to function independently under minimal supervision.
Physical Requirements
- May encounter individuals with aggressive behavior.
This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
