Shared Health leads the planning and coordinates the integration of patient- centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.
Position Overview
Under the general direction of the Manager of Health Services, the Transition Coordinator works with renal patients to facilitate care transitions between dialysis modalities and other Kidney Health services. The Transition Coordinator takes a home first approach to renal care and works with patients, their families, and the interdisciplinary Kidney Health team to overcome barriers to home dialysis. Using a patient centred approach, this role provides education to patients with advanced kidney disease nearing the need for or on dialysis modalities to support informed decision making.

Experience
- Experience working in kidney health and dialysis for at least 2 years required.
- Knowledge of kidney health, peritoneal dialysis and hemodialysis required.
- Experience in home dialysis modalities would be an asset.
- Knowledge and working experience with the electronic health records.
- Experience and training in motivational counseling would be an asset.
Education (Degree/Diploma/Certificate)
- Bachelor of Social Work (BSW) or Bachelor of Nursing (BN) required.
- Completion of Manitoba Indigenous Cultural Safety Training (MISCT) preferred and will be required as part of the position.
Certification/Licensure/Registration
- Valid licensure with applicable regulatory body.
- Subject to criminal record and abuse registry checks.
Qualifications and Skills
- Superior communication and interpersonal skills.
- Demonstrated ability to work independently as well as collaboratively with patients, families, and interdisciplinary team members.
- Demonstrated ability to facilitate patient transfers between health care facilities and renal units as well as between the various modalities used to treat kidney failure.
- Demonstrated ability to establish and maintain effective working relationships.
- Demonstrated ability to establish and maintain effective resource contacts.
- Ability to build strong relationships and support patient voice with leadership, engagement and empowerment.
- Ability to travel to patients’ homes and the various health care and long-term care facilities.
- Knowledge and experience with computers and proficiency in Microsoft Office software.
- Demonstrated knowledge and skills in research preferred.
Physical Requirements
- Valid Manitoba Class V Driver’s License; access to a vehicle and willingness to travel throughout the province.
- Ability to work flexible hours in order to carry out the duties and responsibilities of the position.
