Position Overview

The Home Care Program provides a broad range of services to assist people to remain in a community setting. The Hospital Based Pediatric Case Coordinator (PCC) receives referrals and conducts assessments to determine eligibility for supports related to Home Care and Integrated Children’s Services (care needs related to both Children’s Home Care and Children’s disability Services). The PCC in collaboration with the family and relevant interdisciplinary team members, develops, coordinates and evaluates the plan of care. The PCC provides professional intervention where appropriate and maintains ongoing management of cases. Additionally, the PCC establishes and maintains liaison with local health care services, hospital, community partners, children and their families. The PCC works in an integrated service delivery model for the provision of services for children who are eligible for supports from Home Care and/or Integrated Children’s Services.

Experience

  • Minimum three years recent related nursing experience required (specific recent experience may be required in designated areas).
  • Two years directly related pediatric experience required.
  • Experience with Hospital Based Case Coordination an asset.
  • Working knowledge of the WRHA Home Care programs an asset.

Education (Degree/Diploma/Certificate)

  • Graduate of an approved Registered Nursing education program required.
  • BN,BScMH is preferred.

Certification/Licensure/Registration

  • Responsible for maintaining and providing proof of registration with the College of Registered Nurses of Manitoba (CRNM).
  • Cardiopulmonary Resuscitation (CPR) Training requirements for this position shall be in accordance with Employer policy.
  • Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) preferred.
  • Valid Driver’s License and use of a vehicle.

Qualifications and Skills

  • Knowledge of:
    • Working knowledge of the WRHA Home Care programs an asset.
    • College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses.
    • Canadian Nurses Association Code of Ethics for Registered Nurses.
    • Scope of practice document as documented in the Registered Nurses Act.
    • Regional/Facility unit policies, procedures, protocols, and guidelines.
    • Nursing care knowledge related to community nursing.
    • Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions) and other legislated acts.
    • Roles and responsibilities of members of the healthcare team.
    • Principles of delegation.
    • Principles of adult learning and teaching strategies.
  • Knowledge and experience with computers.
  • Demonstrated effective oral and written communication skills.
  • Preference will be given to those applicants competent in an Indigenous language and/or knowledge of Indigenous customs, traditions and values.
  • Demonstrated critical thinking/problem solving skills Ability to respond to a variety of simultaneous demands.
  • Ability to recognize and pursue self-development opportunities.
  • Ability to perform independently and as a member of the healthcare team.
  • Ability to adapt quickly to changing situations.
  • Ability to maintain concentration with frequent interruptions.

Physical Requirements

  • Available to work days, evenings and weekends.
  • Use of a reliable motor vehicle suitable for all environmental conditions.

 

 

This term position may end earlier as outlined in your collective agreement.

 

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment.
