Position Overview

PROGRAM SERVICE OVERVIEW 
The Rehabilitation, Healthy Aging& Seniors Care Program provides equitably accessible services, focusing on client strengths and needs, and problem-solving for client-focused solutions. Geriatric Mental Health clinicians do assessments in the homes of people 65+ and in personal care homes within the WRHA. They work closely with Geriatric Specialist Psychiatrists They make recommendations to primary care providers, and Personal Care Home teams, and coordinate connection with services for clients focusing on their individual needs.  The clinicians use a multidimensional mental health assessment and a framework of advanced geriatric practice in order to function as clinician, coordinator, communicator, and advocate in both community and Long Term Care settings.

MAIN FUNCTION:    
The Geriatric Mental Health Clinician contributes to the health care of geriatric patients (high-risk frail adults 65+), by using a comprehensive multidimensional assessment, planning, coordination and evaluation.  The Geriatric Mental Health Clinicians contribute to excellence in care throughout the system by helping long term care teams to problem solve and build capacity for management of complex cases in Personal Care Homes. They also provide support to care providers and clients to maximize health, safety and independence in the community.

Experience

  • Minimum of three years geriatric and /or mental health experience required.
  • Ability to work with minimal supervision as well as collaboratively in a team.

Education (Degree/Diploma/Certificate)

  • Entry-to-practice academic credential in a relevant health care profession from an approved Canadian university program or the approved equivalent if internationally educated.  Relevant health care related Master’s degree preferred.
  • Must successfully complete individualized self-directed geriatric training provided through the program to each clinician in order to independently complete a comprehensive geriatric mental health assessment.

Certification/Licensure/Registration

  • Current registration/licensure with a Manitoba Health care professional licensing body (CRNM, COTM, MCSW).
  • Valid Manitoba Driver’s License and access to a vehicle required.

  • Ability to establish good interpersonal relationships with clients and health care professionals
  • Effective written and verbal communication skills

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
