Position Overview

The Geriatric Program Assessment Team conducts a multidimensional geriatric assessment, evaluating individual client strengths and needs, and problem-solving for client-focused solutions. Geriatric Clinicians do assessments in the homes of people 65+ and provide geriatric consultation services to the Emergency Departments (ED) and Urgent Care (UC) departments within the WRHA. In the ED/UC, geriatric clinicians facilitate direct transfer of clients into geriatric rehabilitation units as appropriate. Geriatric Clinicians work very closely with the Geriatric Specialist Physicians They make recommendations to primary care providers, and coordinate connection with services for clients focusing on their individual needs. The clinicians use a multidimensional assessment and a framework of advanced geriatric practice in order to function as clinician, coordinator, communicator, and advocate in both hospital and community settings.

The Geriatric Clinician contributes to the health care of high-risk frail adults 65+, by using a comprehensive multidimensional assessment, planning, coordination and evaluation. Geriatric Clinicians contribute to patient flow within the health care system by facilitating transfer of clients to geriatric rehabilitation units, and by providing community follow-up of clients on discharge from the ED/UC and inpatient care units. GPAT provides support to care providers and clients to maximize health, safety and independence in the community.

Experience

  • Minimum of three years geriatric clinical experience.
  • Ability to work with minimal supervision as well as collaboratively in a team.

Education (Degree/Diploma/Certificate)

  • Baccalaureate degree in Nursing, Occupational Therapy, Physical Therapy, Social Work or other related health care disciplines required.
  • Clinical Master's degree preferred.
  • Must successfully complete individualized self-directed geriatric training provided through the program to each clinician in order to independently complete a comprehensive geriatric assessment.

Certification/Licensure/Registration

  • Current registration/licensure with a Manitoba Health care professional licensing body (CRNM, COTM, CPM, MCSW).
  • Valid Manitoba driver’s license and access to a vehicle required.

Qualifications and Skills

  • Community based work experience.
  • Self-directed.
  • A demonstrated interest in geriatrics.
  • Ability to establish good interpersonal relationships with clients and health care professionals. 
  • Effective written and verbal communication skills.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment.
