Position Overview
The Community Services Coordinator functions as a member of the My Health Team to respond to requests to assist with the health and social needs, including mental health needs of marginalized, at-risk populations and the agencies that serve them. The Health Team is a population resource for clients connected to partner Fee-for-Service Physician practices and Community Health Organizations that provide primary care services within a community area. This includes facilitating seamless transitions and service integration between community partners —such as emergency shelters and Housing First programs —and the MyHT partner clinics to ensure continuity of primary care.
The Community Services Coordinator provides outreach assistance in consultation requests, coordinated service planning, capacity building efforts, and direct service needs to implement individual plans or access health and social service resources. Through development of supportive relationships, the Community Services Coordinator provides tangible assistance in a manner that aims to increase individual capacity and foster mental health. The Community Services Coordinator may provide support assistance to bridge service gaps by collaborating in plan development and providing assistance to implement action plans including supporting connections to services and resources. The Community Coordinator may assist individuals to connect to a range of services including services for: obtaining identification, specialized mental health treatment, crisis response service, general health services, housing access, income support, protection and advocacy, and connections to family, peer and community networks. The Community Services Coordinator will provide comprehensive knowledge in mental health resources and how to connect.
The Community Services Coordinator adheres to the documentation, consent and privacy standards of the Winnipeg Regional Health Authority. The Community Services Coordinator ensures that s/he performs her/his duties in a manner which is consistent with the standards of the appropriate professional/ licensing body.
Experience
- Minimum two years of experience working with marginalized populations, including individuals experiencing homelessness and mental health issues.
- Experience supporting individuals to realize individual goals and access services as required.
Education (Degree/Diploma/Certificate)
- Recognized post-secondary certificate and or diploma in a health services or social sciences field required.
- Health care degree preferred.
- Non-violent Crisis Intervention Training.
- ASIST Certificate (Applied Suicide Intervention Skills Training).
Certification/Licensure/Registration
- Member in good standing of relevant professional organization.
- Valid driver’s license and access to a vehicle.
Qualifications and Skills

- Ability to work cooperatively with other disciplines.
- Ability to perform both independently and as an integrated community team member, including effective communication with team manager.
- Demonstrated effective organizational, interpersonal, critical thinking, clinical reasoning, conflict resolution and decision-making skills.
- Experience and willingness to learn and use new concepts, techniques and best practice.
- Experience with development and/or support of person-centred plans for individuals with mental health issues.
- Knowledge of available health and social service resources for referral.
- Experience with providing capacity building support.
- Experience providing supportive counselling.
- Knowledge of Indigenous cultural history and population impacts.
- Knowledge of and commitment to the principles of population public health.
- Knowledge of and commitment to the principles of Harm Reduction.
- Knowledge of and commitment to the principles of Housing First.
- Knowledge of and commitment to the principles of Trauma-Informed Recovery.
- Knowledge of and commitment to Psychosocial Rehabilitation and Recovery
- Computer literacy for communication, word processing and utilized clinical applications.
Physical Requirements
- Must be able to provide service in a mobile, outreach capacity.
- May be exposed to physical and emotional stress.
- May encounter aggressive and/or agitated clients/visitors/staff.
This term position may end earlier as outlined in your collective agreement.
This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
