Position Summary:
This Community Health Nurse provides direct nursing care to individuals living in the West Broadway and Downtown area who access primary care at Klinic. The nurse in this position will function in the role of Outreach Nurse. This role includes providing direct care to clients in their homes including but not limited to physical assessment (respiratory, cardiac, wound care), venipuncture, foot care, immunizations, and INR management. They may also assist with Activities of Daily Living (ADL) and system navigation (social and medical). Experience supporting clients on opiate agonist therapy (OAT) will be an asset in this position. Transportation of clients may be required.
QUALIFICATIONS:
- Minimum of 3 years’ experience in Community Health and/or Primary Care
- Experience in Gerontology, Home Care and Mental Health is an asset
- Minimum of 5 years nursing experience preferred
- Bachelor of Nursing preferred (equivalent combination of experience and education may be considered)
- Active registration in the College of Registered Nurses of Manitoba or equivalent
- Knowledge of PHIA legislation and its application
- Demonstrated clinical skill in physical assessment and nursing intervention
- Demonstrated ability to work independently and collaboratively as a member of a multi-disciplinary team
- Current CPR certification
- Valid driver’s license and access to a vehicle is required
- Knowledge of community resources
- Working knowledge of basic computer usage and Microsoft Office
- Experience with electronic medical records (particularly Accuro) is an asset.
- Demonstrated effective communication skills
- Demonstrated ability to contribute to a positive work environment.
Responsibilities:
- Promotes equity to high risk clients (e.g. elderly, adolescent, mental health diagnosis) and groups experiencing lower health status due to multiple social health determinants (e.g. lower income, education, language barrier, housing instability).
- Applies culturally-relevant and appropriate approaches with people from diverse cultural, socioeconomic and educational backgrounds, sexual minorities, and persons of all ages, genders, health status and abilities.
- Documents accurate and pertinent client information in a timely manner.
- Provides information, educational support and/or advocacy to clients
- Works as part of an interdisciplinary team
- Provides case management and leads team, acting as a first contact in the collaborative care of high needs clients.
Other:
- Assists in the orientation and training of new staff members and students.
- Participates in training courses and in-services offered elsewhere and by Klinic in order to maintain and improve quality of service.
- Attends staff meetings as required
- Works in conjunction with all Klinic staff and external agencies as is necessary to ensure a collaborative approach to meeting health care needs
- Participates in program planning, evaluation and quality monitoring including completing daily statistics
- Participates in formal self-evaluations by identifying strengths and limitations
- Seeks appropriate supervision/consultation of own actions
- Adheres to all agency policies, procedures, protocols and promotes best practice
- Performs other duties and functions related to their job description
- Subject to immunization and tuberculin skin testing as per Klinic policies
Working Environment and Physical Abilities:
- Busy medical clinic
- Nursing care will be provided off-site in client homes (potential allergens or other hazards), community meeting spaces, or in specialty clinics (e.g. STI Klinic)
- Smudging occurs at least weekly at the main Klinic Site
- Must be capable of performing duties related to the position, including assisting clients with physical and mental challenges, lifting clinic equipment and teaching materials and arranging meeting rooms for groups.
SALARY RANGE $43.321 to $53.238 per hour
