Position Overview

Responsible for the delivery of social work services to all residents as well as their representatives including psycho-social support, assessments and interventions with PCH placement. Coordinates all admissions (and discharges) to and from the facility, manages bed utilization to ensure the best use of available beds and maximizing occupancy. Promotes and maintains a positive, professional, team orientated and service conscious environment to support the twenty-four (24) hour delivery of safe, quality resident care. Provides counseling to residents and families on an ongoing basis.

Experience

  • Two (2) years’ experience in a PCH setting preferred.
  • Experience in counseling and group work with the elderly and families an asset.

Education (Degree/Diploma/Certificate)

  • BSW from an accredited University.

Certification/Licensure/Registration

  • Registration with MIRSW/MASW.

Qualifications and Skills

  • Effective oral and written communication skills.
  • Ability to handle multiple projects and requests in an organized and timely fashion.
  • Working knowledge of RAI/MDS.
  • Functions within a team concept.
  • Demonstrated ability to be creative, assertive and innovative.
  • Commitment to excellence in care of the elderly.
  • Familiar with operating in a computerized environment.
  • Demonstrated ability to communicate in English, both orally and in writing.

Physical Requirements

  • Must be flexible.
  • Must be self-directed and have the ability to work in a rapidly changing internal and external environment.
  • May be required to work extended hours.
  • May encounter aggressive and/or agitated individuals.
  • May encounter exposure to disease, radiation, toxic material, noise, allergens, etc.

 

 

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
