Position Summary:

The Housing & Supports Social Worker will work directly with supportive housing program participants living with mental illness in an integrated community setting. The Housing & Supports Social Worker will complete mental health assessments, collaborate with clients to maintain tenancy and achieve their goals, participate in care planning and crisis intervention, and facilitate family/support meetings. This position also involves maintaining communication with relevant agencies, organizing workshops and therapeutic groups, supporting activities of daily living (ADL’s), and connecting individuals with community resources.

 

Qualifications & Experience:

  • Bachelor’s degree in Social Work required

  • Current Social Work license/registration with Manitoba College of Social Workers required

  • Previous experience working with a residential program is considered an asset.

 

Knowledge, Skills and Ability:

  • Excellent crisis intervention and conflict mediation skills, with ability to remain calm in crisis.

  • Knowledge of psychiatric symptomology and suicide assessment and intervention

  • Cultural competence and a demonstrated ability to adapt interventions to ensure equity, access, and accommodation. 

  • Knowledge of community resources relevant to the target population being served and/or demonstrated ability to search for applicable resources/information

  • A broad understanding of all factors that impact an individual’s mental health and well-being, including the social determinants of health, trauma, poverty and homelessness.

  • Personal qualities of empathy, understanding, patience, and flexibility.

  • Excellent rapport-building skills.

  • Working knowledge of harm reduction, person-centred practice, trauma informed care, and recovery principles and practice.

  • Demonstrates skills in therapeutic interventions – individual, family and group.

  • Has a working knowledge of the Mental Health Act, including confidentiality requirements.

  • Familiar with the network of community agencies and resources available to participants.

  • Excellent verbal and written communication skills

  • Excellent computer skills with proficiency in all Microsoft programs

  • Has a working knowledge of respectful workplace requirements and demonstrated ability to maintain positive working relationships.

  • Excellent organizational, prioritization and time management skills.

  • Ability to work both independently and as part of a team.

  • Committed to ongoing personal and professional development.

 

Other Requirements and Conditions of Employment:

  • Satisfactory Criminal Record Check (with the Vulnerable Sector Check).

  • Satisfactory Child Abuse Registry Check and Adult Abuse Registry Check.

  • Valid Driver’s License.

  • Current Vehicle Registration, indicating 3rd Party Liability Insurance of $1,000,000 or more.

  • Valid registration with Manitoba College of Social Workers.

 

Principal Duties and Responsibilities:

  • Completing regular mental health assessments using the RAI-CMH tool.

  • Obtaining collateral information and social histories for assessment purposes.

  • Working collaboratively with clients to identify, develop, and fulfill goals they identify as meaningful and relevant.

  • Consulting, collaborating, and participating in meetings with the participants’ supports, to plan, inform and review care plans in relation to participants goals.

  • Assisting participants with access to benefits through gathering information and completing forms and applications as required.

  • Collaborating with participants to find meaningful daytime activities including employment and/or volunteer opportunities.

  • Providing support to de-escalate and stabilize crisis.

  • Maintaining ongoing communication with CMHW, PACT, Public Guardian and Trustee, and all other agencies involved in participant’s care.

  • Arranging and facilitating family/support network meetings, which may include:

    • preparing the agenda

    • informing the team members of the agenda and goals for the meetings

    • chairing meetings as appropriate

  • Completing all required documentation.

  • Discharge/transition planning as directed.

  • Developing and facilitating therapeutic groups, which includes reviewing group material routinely with the Program Development Coordinator to algin with evidence-based practice.

  • Arranging workshops in partnership with other Eden staff or community organizations.

  • Assisting with self-sufficiency in Activities of Daily Living (e.g., cooking, cleaning, personal hygiene) as needed.

  • Supporting participants to identify and access needed community resources, as appropriate, in collaboration with the support team.

  • Navigating access to resources using referral, direct support (e.g., accompaniment), resource arrangement, and advocacy.

  • Upholding and reflecting the values of Eden Health Care Services (hope, integrity, excellence, compassion, respect, and acceptance). 

  • Participating in the orientation of new staff. 

Other Duties:

  • Consulting with supervisor on a regular basis.

  • Attending in-services and other educational events as part of mandatory training and as other opportunities arise; continuing competency.

  • Participating in committee work as required.

  • Supervising Social Work students and acts as a preceptor as opportunities arise.

  • Other duties as assigned.
