Position Overview

The Community Health Nurse (CHN) is accountable for the delivery of nursing care to individuals or groups of clients and is an integral member of a collaborative, multidisciplinary primary care team. Guided by the principles of primary care and public health, the Community Health Nurse provides comprehensive care to individuals, families and communities with an emphasis on healthy living, illness prevention, health education, chronic disease management, case management, clinical intervention, and palliation.

 

The Community Health Nurse may practice in a variety of settings, including primary health care centers, mobile/temporary clinics in community settings, correctional facilities, and/or within a primary care or medical clinic. The focus is on a shared client population with Nurse Practitioners, Primary Care Physicians, and allied health in a collaborative primary care practice model. Depending upon the needs of the community, the incumbent may also be responsible for public health nursing roles of population health promotion and health protection.

 

The incumbent exercises the appropriate level of initiative and independent judgment in determining work priorities, work methods to be employed and action to be taken on unusual matters. The position functions in a manner that is consistent with the mission, vision and values; and the policies of Southern Health-Santé Sud.

Experience

  • Minimum of two (2) years full time equivalent experience in a community health nurse position in the last five (5) years.

Education (Degree/Diploma/Certificate)

  • Baccalaureate Degree in Nursing.

Certification/Licensure/Registration

  • Current active registration with the College of Registered Nurse of Manitoba and the scope of practice as defined by the Regulated Health Professions Act (RHPA).
  • Basic Life Support.
  • Additional certification in specialty area (e.g.: Immunizations, Harm reduction, Trauma Informed Care, MISCT, Testing and treating of STBBI -specimen collection and in scope treatment options, Certified diabetes educator, certified tobacco educator, certified asthma educator, or wound care) is an asset.
  • Requires a valid Class 5 driver’s license, an all-purpose insured vehicle and liability insurance of at least $1,000,000.00.

Qualifications and Skills

  • Knowledge of:
    • Professional Standards of Practice, and Code of Ethics.
    • Public Health Act, required.Clinical assessment care and treatment, health education and promotion, illness prevention, and chronic disease management in adults, youths and pediatrics.
    • Principles of primary care, population public health, determinants of health, and harm reduction.
  • Demonstrated ability to:
    • Maintain both professional nursing standards and community health nursing standards.
    • Apply adult learning principles to both individual and group education models of care.
    • Integrate and apply both behaviour change and harm reduction concepts.
    • Engage in inter-professional practice and collaborate with members of the multi-disciplinary health care team.
    • Respond appropriately in crisis situations.
    • Establish and maintain positive working relationships.
    • Work independently.
    • Respect confidentiality including paper, electronic formats and other mediums.
  • Demonstrated:
    • Effective collaboration, negotiation and conflict resolution skills.
    • Effective oral and written communication skills.
    • Organizational skills and ability to prioritize work effectively.
    • Decision making and problem-solving skills in complex situations.
    • Adaptability to facilitate changes in techniques and procedures in a changing environment.
  • Able to effectively perform the tasks and responsibilities of the position.
  • Proficiency with Microsoft Office and Electronic Medical Record applications.
  • Given the cultural diversity of our region, the ability to respect and promote a culturally diverse population is required.
  • Proficiency of both official languages is essential for target and designated bilingual positions.
  • Good work and attendance record.

Physical Requirements

  • Demonstrated ability to meet the physical and mental demands of the job.
  • No hazardous or significantly unpleasant conditions.
  • May work occasionally evenings and weekends as necessary.
  • Will be required to travel to other regional facilities as the position duties may require.
