Shared Health leads the planning and coordinates the integration of patient-centred clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.

Position Overview

The Patient Navigator provides field-based case management services to clients enrolled in the ‘Out-of-Province’ program. This person is the main point of contact for clients. The Patient Navigator builds strong relationships with surgery clients in order to help clients stay engaged in medical care and adhere to their medications. Patient Navigators are committed to removing the client’s barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health. They work closely with the Care Team, which may include doctors, nurses, and other clinical staff to support positive client health outcomes.

Experience

  • 3-5 years previous related experience in hospital utilization and surgical informatics required.
  • Previous experience in operating room and pre-admission environment required. 
  • Knowledge of hospital operations, health care programs and patient care delivery systems preferred. 
  • Incumbent is required to be self-motivated with excellent problem-solving, good judgment, organization, communications, interpersonal and negotiating skills. 
  • Must be able to function with limited guidance, supervision and monitoring.  

Education (Degree/Diploma/Certificate)

  • Complete high school education, Manitoba standards required. 
  • Certificate in Health services administration, surgical health informatics or a related discipline. 
  • An equivalent combination of education and experience may be considered.

Certification/Licensure/Registration

  • Enrollment in recognized health services administration certificate or equivalent education and experience.

Qualifications and Skills

  • Demonstrated proficiency with computerized systems (Microsoft Office Word, Outlook, Excel, Access, PowerPoint, and Visio Software required.
  • Knowledge of SIMS (Surgical Information Systems) required. 
  • Must be able to function independently and commutate effectively with all levels of staff, government, media and the public. 
  • Medical terminology required.
  • Experience with surgical workflow, process and nomenclature. 
  • Ensuring seamless scheduling and facilitating efficient use of operating room time. 
  • Maintaining communication between clients and external providers.
  • Develop and maintain data base and reports.

Physical Requirements

  • Ability to cope with unexpected interruptions in order to meet challenging deadlines. 
  • Good physical and mental health required.
